E. Coli Infection: How to Stay Safe

Escherichia coli,鈥痮谤鈥疎. coli,鈥is a type of bacteria commonly found in human and animal intestines. Most E. coli strains are harmless, says Dr. Heidi Torres, Assistant Professor of Clinical Medicine in the Division of Infectious Diseases and Assistant Hospital Epidemiologist at 麻豆传媒高清版, and they鈥檙e even a beneficial part of the gut鈥檚 microbial ecosystem. However, certain strains can cause illness, with symptoms including diarrhea, urinary tract infections and pneumonia.

Which E. coli strains are responsible for the two E. coli outbreaks this fall?

E. coli鈥疧157:H7, also known as enterohemorrhagic E. coli, can lead to intestinal infections accompanied by bloody diarrhea, among other severe symptoms. In a recent outbreak identified this 翱肠迟辞产别谤,鈥E. coli鈥疧157:H7 was traced to sliced onions served at various McDonald's locations, resulting in 104 known cases across 14 states, including New York.

Another strain,鈥E. coli鈥疧121, can cause similar symptoms. That strain was responsible for a separate outbreak identified in mid-November, linked to bagged organic carrots, with 39 known cases across 18 states reported thus far.

However, according to the Centers for Disease Control and Prevention (CDC), the actual number those made sick by the two E. coli strains is probably significantly higher. That鈥檚 because some people improve on their own and aren鈥檛 included in the data.

How can people become infected with a harmful type of E. coli?

People typically become infected with harmful strains of鈥E. coli鈥痶hrough contaminated food and water,鈥 Dr. Torres says. During processing, meat may be contaminated with鈥E. coli鈥痜rom cattle intestines; the bacteria can survive if your food isn鈥檛 cooked thoroughly.

鈥淯npasteurized milk can also carry鈥E. coli鈥痠f it鈥檚 contaminated from the cattle,鈥 she adds. 鈥淎s well, water sources exposed to human or animal waste can transmit bacteria to vegetables and other foods or directly infect individuals who come into contact with contaminated water. Close contact with an infected person can further spread鈥E. coli鈥痓补肠迟别谤颈补.鈥

Who is especially vulnerable to infection?

As with many infections, young children, older adults and people with weakened immune systems are at greater risk of contracting鈥E. coli鈥痑nd experiencing severe illness.听

鈥淎dditionally, taking antacids puts you at increased risk, as stomach acid plays a role in killing swallowed bacteria,鈥 she says.

What are the most common symptoms of an E. coli infection?

Symptoms usually begin within a few days and often include stomach cramps, diarrhea (possibly bloody), fatigue, nausea and vomiting. Rarely, she says, 鈥渁n鈥E. coli鈥痠nfection can lead to hemolytic uremic syndrome (HUS), a serious complication that may cause kidney failure and can be life-threatening.鈥

How do you typically treat patients with an E. coli infection?

Treatment for an鈥E. coli鈥痠nfection involves supportive care, including fluids, electrolytes and rest. These are usually enough to ensure recovery.听

鈥淎ntibiotics and antidiarrheals are not routinely recommended and should be avoided for certain E. coli strains鈥--including the two current 鈥榯roublemakers,鈥 O157:H7 and O121. Paradoxically, these medications may increase the risk of HUS.

For other鈥E. coli鈥痵trains with a lower risk of HUS, antibiotics may be administered to those with severe or prolonged di邪rrh械邪 or those at risk for severe disease. The distinction between strains can often be made through stool testing.

When is it time for a patient to call their doctor?

Patients should contact their doctor if they experience persistent or bloody diarrhea; frequent vomiting that prevents fluid intake; or signs of dehydration, such as a rapid heart rate or lightheadedness,鈥 Dr. Torres says.

What are the best ways to protect ourselves from infection?

To prevent鈥E. coli鈥痑nd other food- and water-borne infections, follow these recommendations:

  • Cook meat to the appropriate internal temperature.
  • Avoid unpasteurized juices or milk.
  • Thoroughly wash all fruits and vegetables with water.
  • Drink only safe, potable water.
  • Avoid swimming in water that may be contaminated with animal or human waste.
  • Practice good hygiene by washing your hands thoroughly after using the bathroom.
  • Avoid handling food if you have diarrhea.
  • Stay informed about food recalls.

Any final comments about the outbreak?

The onions linked to the鈥E. coli outbreak were distributed to food businesses and are not expected to have been found in grocery stores.These onions have passed their shelf life and are no longer available for sale. In other words, the source of that outbreak is no longer a threat.
