What Parents Should Know About Tonsillectomy and Adenoidectomy
Tonsil and adenoid surgeries听are听the most common surgical procedures听performed on children in the United States.听Tha迟鈥檚 been true听for about the last 20 years, says听Vikash K Modi, MD,听Chief of Pediatric Otolaryngology - Head and Neck Surgery听at 麻豆传媒高清版. Dr. Modi, also听Associate Professor of Otolaryngology in Pediatrics at Weill Cornell Medical College, reports that about 75 percent of his practice comprises pediatric tonsil听and adenoid surgery.听听

Tonsil and adenoid response to infections
Tonsils are located on both sides of the back of the throat,听and adenoids are located in the back of the nose.鈥听
鈥Often, in response to infection,听or for other reasons, the tonsils will enlarge,听and you can see them as two 鈥痩arge鈥 balls of tissue on either side of the throat,鈥 Dr. Modi explains.鈥赌听听
Adenoid tissue may enlarge in response to respiratory tract infections, allergies, or听sometimes听for unknown reasons.听
Common reasons for tonsil and adenoid surgeries
There are two听common reasons听for tonsil and adenoid surgery:听infection and upper airway obstruction.鈥听听
Frequent throat infection鈥攎ost commonly, strep throat鈥is an indication for tonsillectomy听when a听child has had seven or more episodes in one year, 鈥痜ive episodes in two successive years,鈥 or three episodes in three successive years.鈥赌听
Physicians may recommend a partial鈥痮r total鈥痶onsillectomy听when the tonsils and/or adenoids become enlarged and听obstruct the听airway.听听
鈥淧arents should watch for disruptive sleep patterns, such as frequent waking at night听that听occurs听with snoring, or apnea听[a cessation of breathing],鈥 Dr. Modi says. 鈥Sometimes parents听hear loud, irregular snoring at night,听or听notice their child is wetting听the bed听or听mouth-breathing.鈥听
Enlarged adenoids may cause chronic earaches and ear infections, with or without middle ear fluid.听听
鈥Adenoid tissue is near the opening of the鈥 eustachian 鈥痶ube,听鈥痺hich 鈥痸entilates the middle ear space,鈥 Dr. Modi says. 鈥鈥Large adenoids can鈥痓lock the鈥 eustachian鈥 tube 鈥痮pening, leading to fluid build-up, ear infections, and hearing loss.鈥鈥赌听听
A听doctor may recommend an adenoidectomy鈥for a child who experiences long-term or recurrent sinus or nasal infections despite adequate medical treatment听he听adds.听听
When parents notice that their child is more tired than usual or听is听taking听daytime听naps, it can be a听sign of possible disrupted sleep and/or sleep apnea.听听听
鈥淎pnea听can affect a child's growth and development,鈥 Dr. Modi says.鈥鈥Disrupted sleep and/or sleep apnea鈥also听丑补蝉鈥been closely correlated with learning disabilities,听such as ADHD.听Long-term听sleep apnea can lead to cardiopulmonary issues.鈥听
Surgical options can include tonsil reduction (shaving)听or removal.听
If a child suffers from听sleep apnea or obstruction,听surgeons would typically听perform a听partial tonsillectomy听with听an听adenoidectomy.鈥When the major challenge is听chronic听throat鈥痠nfections, they鈥檇 be more likely to perform鈥痑 total tonsillectomy, Dr. Modi says.鈥赌听听
I迟鈥檚 important to prepare听a听child for these procedures
Sometimes children may be given a sedative before surgery, and anesthesia is administered with a mask. Both may surprise children, so Dr. Modi advises parents to prepare their child for surgery.听
Parents are allowed to be with children听in the operating room until听the听child is听asleep, and听in the recovery room when听the听child wakes up.鈥听
鈥There are several books that you and your child can read together that would help to prepare for the experience,鈥 Dr. Modi adds.鈥鈥One example is听Curious George Goes to the Hospital, by M & H Rey.鈥澨听
Tonsillectomies and adenoidectomies are usually outpatient procedures
Sometimes, Dr. Modi explains, a听child may stay in the hospital overnight.听听
Both procedures鈥攚hich require general anesthesia鈥攁re brief. An adenoidectomy usually takes 30 to 40 minutes, and the surgeries performed together usually take between 45 and 60 minutes.听
鈥淎fter a tonsillectomy, children may experience some pain and bleeding,鈥澨Dr. Modi听says. Bleeding is less frequent with a partial听tonsillectomy, and usually occurs closer to the time of the procedure, or about a week later, when a scab falls off the surgical site.听听
With a partial鈥痶onsillectomy,鈥 a child may complain of a mild sore throat for听two to three听days after surgery. Dr. Modi says over-the-counter鈥 medications usually听bring relief.听
Recovering from surgery
Some children feel better in just a few days, and some take as many as 14 days to recover.鈥赌听
鈥淒uring the recovery period, make sure your child stays well hydrated,鈥 Dr. Modi says, and suggests听avoiding carbonated drinks, and acidic drinks (like orange juice).听听
A听soft diet where chewing is necessary听also is important: chewing and swallowing relieve pain and promote healing.听
Parents may notice听a听high-pitched or squeaky听change in听the child鈥檚听voice鈥i迟鈥檚听usually temporary,听but may last听for听six听to听eight听weeks.鈥听
Children who undergo听a partial tonsillectomy听usually听return to school when鈥 they 鈥痜eel鈥 ready,鈥 usually听in three to five听days.听听
